They will generally have one or more rectangular boxes where you input your name or email address or phone number etc. This opt-in form or box will be inserted into a webpage sometimes called a capture page or splash page or landing page or it might be inserted into a blog. You will send your prospects to these pages and instruct them to insert their contact information. Once they enter their details and press the submit button they are automatically subscribed to your email follow-up campaign.
